New Patient Forms

Dr. Robertson needs your complete health history to correctly diagnose any problems you may have, and to choose the most effective care plan to get you feeling better as quickly as possible. Please print the following forms, fill them out completely, and bring them to your first visit.

NOTE: ALL new patients will need to complete the New Patient Forms packet. This includes the following:

New Patient History
HIPAA (Privacy)
Financial Policy
Missed Appointment Policy

Patients who have been in an accident ALSO need to fill out the Automobile Accident/Personal Injury Forms or Worker's Compensation Forms, as appropriate. Please let our staff know immediately about either of these situations when scheduling your initial appointment.
