PATIENTS WHO HAVE BEEN IN AN ACCIDENT also need to fill out the Automobile Accident/Personal Injury Forms or Worker's Compensation Forms, as appropriate. Please let our staff know immediately about either of these situations when scheduling your initial appointment.
